Case Manager

Description:

Major Responsibilities/Activities:

Assessment of clients needs, including activities of daily living, at least every 90 days.
Referral and linkage to resources and providers who can meet client’s needs.
Treatment (Care) planning with client and others involved in their care. Includes friends and family members who the client would like to involve, at least every 90 days.
Present yourself and Winds of Change in a professional manner in the community.
Complete progress notes daily.
Complete internal and external forms and other required documents by the timeline.
Complete required training (in person and / or by computer based learning system)
Must have current driver’s license, transportation, and automobile insurance coverage.
Follow HIPAA and maintain confidentiality.
Maintain productivity requirement.
Attend weekly scheduled meetings as required.
Other duties as assigned.
Ability to maintain regular attendance.
