Position Requirements: Requirements: Compliance with HIPAA regulations. High level of confidentiality and discretion. Proactive and anticipates needs. Exhibit sound and accurate judgment. Passionate about your work and positive attitude. Takes personal responsibility for own actions, keeping commitments and asking for/offering help when needed. Demonstrates professional appearance and hygiene in accordance with established workplace culture. Education: Bachelor’s Degree in Social Work, Psychology, or a related field is required. For applicants with a Bachelor’s Degree in an unrelated field, one year of experience working in a direct service capacity (i.e., after school program, nursing home, mental health, client advocacy) is required.

Description:

Major Responsibilities/Activities:

Assessment of clients needs, including activities of daily living, at least every 90 days.
Referral and linkage to resources and providers who can meet client’s needs.
Treatment (Care) planning with client and others involved in their care. Includes friends and family members who the client would like to involve, at least every 90 days.
Present yourself and Winds of Change in a professional manner in the community.
Complete progress notes daily.
Complete internal and external forms and other required documents by the timeline.
Complete required training (in person and / or by computer based learning system)
Must have current driver’s license, transportation, and automobile insurance coverage.
Follow HIPAA and maintain confidentiality.
Maintain productivity requirement.
Attend weekly scheduled meetings as required.
Other duties as assigned.
Ability to maintain regular attendance.
